So that we can represent you vis-à-vis other persons or authorities, we require a

If you would like us to provide information to other persons:

  1. Please print out the forms,
  2. provide the power of attorney with names (“in matters” surname, first name),
  3. fill in the consent form for the provision of information, if required,
  4. sign the document(s),
  5. upload the power of attorney and, if applicable, the consent or send it back by email, fax or post.
